SGPGI OPD Online Registration

Registration and appointments are done through the official patient portal at sgpgims.in. First time visitors complete a New Registration to get a reference number and a permanent UHID, which is then used for every future login and visit. Step by step process

  1. Open the official portal at sgpgims.in and go to the Patient Portal or Online Registration.
  2. Read the on-screen instructions and accept the terms to proceed.
  3. For a first visit, choose New Registration and fill in the form.
  4. Enter patient details, address, emergency contact and any referral information.
  5. Select the department and your preferred OPD date, then check department wise availability.
  6. Submit the form with the captcha to receive your reference number, registration date and UHID.
