About SGPGI, Lucknow
The Sanjay Gandhi Postgraduate Institute of Medical Sciences, known as SGPGI or SGPGIMS, is one of India's most respected government super speciality and tertiary care institutes. Set up under a State Legislature Act, it sits on Raebareli Road (PMSSY Road) in Lucknow and runs more than 30 specialised departments, with strengths in fields like cardiology, neurology, nephrology, gastroenterology, endocrinology and oncology.
Because it is a referral and super speciality institute rather than a walk-in general hospital, most OPD visits here work on an appointment basis. OPD runs on working days, and registration is handled in the morning. Since it is a referral institute, follow-up and appointment based visits are the norm.

Each department runs on set weekly days for new and follow-up patients. Old patients are usually asked to come with a prior appointment, so check the department schedule on the portal before travelling.
SGPGI OPD Online Registration
Registration and appointments are done through the official patient portal at sgpgims.in. First time visitors complete a New Registration to get a reference number and a permanent UHID, which is then used for every future login and visit. You can log in with your UHID or reference number plus a mobile OTP.
Step by step process
- Open the official portal at sgpgims.in and go to the Patient Portal or Online Registration.
- Read the on-screen instructions and accept the terms to proceed.
- For a first visit, choose New Registration and fill in the form.
- Enter patient details, address, emergency contact and any referral information.
- Select the department and your preferred OPD date, then check department wise availability.
- Submit the form with the captcha to receive your reference number, registration date and UHID.
- Note or print the confirmation and the token for your appointment day.
- Returning patients simply log in with the UHID and OTP to book a follow-up.
